Dear Patients and Friends:

I have accepted an opportunity to advance my dental education. I will begin a two year program at Boston University specializing in Endodontics on July 1, 2010. My being thrilled at this opportunity comes with significant regret because I will no longer be able to provide dental care for my patients and friends after June 18, 2010.

Your oral health remains my concern and that you receive timely care. You may keep your prescheduled recall appointment in our office and our staff will follow up to confirm that visit. My trusted colleagues at this office are happy to provide your future dental care. Should you wish to transfer your care to another dentist of your choice you may request your records. Since dental records are confidential, your written authorization is required to make them available to another dentist. The same helpful staff who assisted you in the past will continue to serve you.
